Parent Consent Forms

Parents, please check Skyward for new forms for you to complete that will allow the school nurse to see your child for medical reasons and the school counselors to speak with your child about topics such as their schedule, grades, future career plans, or other personal reasons.

Due to the recent passage of Senate Bill 12, if parent consent is not given, the nurse can not consult your child for any reason, nor can counselors provide guidance in any capacity. Thank you for making sure your child is cared for appropriately while at school.
